Shared Care Model
Pregnancy care can begin as soon as you’re pregnant, no referral is required. Lizzie will refer you to hospital for shared care, including labour and birth care. Postnatal care is provided up to 7 weeks post birth. Appointments are bulk billed with Medicare. If you do not have Medicare, please make contact to discuss costs. Care is planned in partnership and aligns with ACM National Midwifery Guidelines for Consultation and Referral and Australian Government Pregnancy Care Guidelines.

Antenatal Appointments
What is an antenatal appointment?
An antenatal appointment is a pregnancy check-up with your midwife to support your health and your baby’s wellbeing throughout pregnancy.
Appointments may include:
• Checking your blood pressure and various symptoms
• Checking your baby’s growth and heartbeat
• Reviewing blood tests and ultrasounds
• Discussing physical and emotional wellbeing
• Pregnancy, birth, and parenting education
• Planning your care and answering questionsAntenatal care is also an opportunity to build a trusting relationship, support informed decision-making, and ensure any concerns are identified early.
Care is individualised to your stage of pregnancy and your individual needs.

Postnatal Appointments
What is a postnatal appointment? 🤍
A postnatal appointment is a check-up with your midwife after your baby is born to support your recovery and your baby’s wellbeing.
It may include:
• Checking your physical recovery and vital signs
• Supporting feeding (breast or bottle)
• Monitoring your baby’s growth, feeding, and wellbeing
• Providing emotional wellbeing support and check-ins
• Checking stitches, bleeding, or other post-birth concerns
• Answering questions and supporting early parentingPostnatal care is also a space to help you adjust, recover, and feel supported in the first weeks after birth.
Care is individualised and based on you and your baby’s needs.
